At Rebel Med NW, we do our best to be candid and transparent in our policies in creating our relationship with our patients. We are your guide and advocate from the smallest bureaucratic details to the most profound health changes that you are looking for.
Office Policies and Procedures
Office visit fees are determined by a combination of the level of complexity, procedures performed, and time spent. Because of this, it is difficult to give an exact amount charged per office visit. However, first office visits are typically between $160 and $280 while return office visits are between $100 and $220. These prices are for patients paying by cash, check, or credit at the time of service and receive the 20% discount for services when not using insurance. Seniors receive an additional Senior’s discount on services. Additionally, there is a sliding scale available for patients who are on a fixed or low income. Additional procedures and codes may be used depending on the complexity of chief complaint and what the practitioner deems is appropriate to achieve the desired health goals of the patient. However, it is important to understand that not all procedures may be covered by your insurance. We will assist patients in this process and help patients with understanding their insurance policies and financial responsibility for commonly used procedures including physical medicine codes and injections, labs and outside lab fees.
Common additional procedures and associated fees are standard medical procedures and are priced accordingly. Some examples include blood draws, vaccinations, vitamin injections, in-house lab testing, outside lab fees, etc. These fees are generally between $20 and $60. Patients paying by cash, check, or credit card at the time of service will receive a 20% discount for services when not using insurance.
Patients have the option to bill their insurance company for their visits and any procedures performed during the visit. Our office provides an insurance verification service as a courtesy to our patients for their convenience. Our office staff do their best to get the most accurate information, however it is important to note that it is the patient’s responsibility to understand their insurance policy and coverage. Having insurance is in no way a guarantee of coverage of all services provided. If we are unable to verify your coverage before the end of your visit, you will be expected to pay in full at the time of your visit for the office visit.
Visits are based on standard medical coding for evaluation and management, which involves both complexity of the case and time spent with the patient. The patient can always submit the payment to the insurance company for reimbursement if coverage was supposed to happen.
Medical insurance, it is a contract between the patient and their insurance company. If a claim is denied, then the patient will be billed for their balance. If the patient has a card on file with our office, then we will be the card that is on record. Not all services or procedures provided by Rebel Med NW may be covered, and the patient will be notified if a service is not expected to be covered and a time of service discount is available for that service.
The providers at Rebel Med NW are in-network with the following insurance companies, however, not all providers are in-network with every company, so please check accordingly. Most insurance companies also have out-of-network coverage available and is a nice option for patients who want to see a specific provider who is not in-network with their insurance. Often, the copay or co-insurance is slightly higher than in-network costs, but still much less than full patient responsibility. Please be aware that in many cases Naturopathic providers and acupuncture providers are considered specialist providers and may require a higher copay. In some cases your employer’s health insurance plan is based in another state, and may not have coverage for naturopathic providers or acupuncture providers despite being fully licensed and credentialed in Washington State.

We have many popular services here at Rebel Med NW, however, they are not always covered by insurance. Some examples include: Craniosacral therapy, Visceral Manipulation, Vitamin Injections, Biofeedback, Spinal Manipulations, Ultrasound, Electrical Stimulation, Massage Therapy, Weight Loss, Acupuncture and Cosmetic Acupuncture, Tui Na Massage, and Herbal medicinals and supplements.
New patient paperwork includes the patient information form, history form, and patient responsibility form and requires about 15 minutes to fill out. You will receive an email with the information to fill out before your initial appointment when an appointment is scheduled 24 hours or more in advanced. If you schedule in less than 24 hours time between your appointment, please arrive 15-20 minutes early to fill out your paperwork ahead of time.
Please bring any relevant medical records along with you on your first visit. A release of records form is available from the office manager to obtain records from other healthcare providers. The form is ideally filled out before or immediately following your first office visit. The process to receive medical records from your previous or current provider can take up to 2-3 weeks so please plan accordingly. Any delays may delay the appropriate course of treatment.
If you have any forms you would like the physician to evaluate prior to your visit, please contact our office. We will provide you with our secure messaging portal when registering as a patient, and you will be able to upload your forms securely.
Prescriptions for nutritional supplements by a Rebel Med physicians are not charged sales tax if they are deemed medically necessary for your care. If a prescription refill has run out or expired, a visit or phone consult is required to determine if the nutritional supplement is still medically necessary. At Rebel Med NW, we believe it is important to have an awareness about what you are taking to minimize the use of excessive or unnecessary supplements and to make sure you are taking the safe, researched, and therapeutic dose that is best for your care.
For Seniors age 65+ and for Medicaid or Income-based Adjustment patients, Rebel Med NW offers a 20% discount on supplement purchases made in office. If a Rebel Med physician has prescribed an item, it may be available on Rebel Med NW’s online dispensary:
In order to expedite requests for prescription refills, please contact your pharmacy and not our office manager. The pharmacy will send us a request via our fax or online via our EHR system, and this will provide us with a necessary paper trail to document the refill. Please allow up to 2 business days to complete request.
Your medical records are kept in the strictest of confidence in accordance with the Health Information Personal Privacy Act (HIPAA). No individuals have access to your medical records without your prior written authorization.
Any medical records cannot be released until a signed Medical Records Release Authorization form is submitted. Please note that although the staff at our office will do their best to fulfill any medical records request as quickly they can, it can take some time. Please allow up to 30 days for medical records to be processed and plan accordingly.
At Rebel Med NW, do our best to accomodate all patient requests in a timely fashion. However, medical bureaucracy takes time and resources, therefore, we do have a Form Processing fee in place. Please note that there is a $26 administration flat fee assessed with an additional $1.17 per page applied to the first 30 pages, and $0.88 per page beyond 30 pages in accordance with the WAC 246-08-400.
The processing fee is applied to help cover administrative costs, costs for duplicating pages, and protecting your privacy when maintaining and transferring medical records. This fee is applied to all large forms processing orders such as sending out medical records requests, filling out medical forms such as Family Medical Leave (FMLA) forms, or any other administrative forms.
All physicians at Rebel Med NW offer free 5-minute phone consultations to answer quick, simple questions for established patients concerning prior authorized care. This courtesy needs to be scheduled with the office in advanced to ensure the physician is available for the phone call. New patient and new treatment authorizations require a full office visit. Any calls over the 5 minutes (within reason) will be considered a full office telemedicine visit.Please call us at 206-297-6013 to schedule consultations and office visits.
The physicians at Rebel Med NW have an online messaging system for our established patients that will reach us in the event of an acute emergency. Since we currently do not provide full primary care services at Rebel Med NW, it is best to contact your Primary Care Provider or 911 if the matter is urgent. If it is not an acute emergency and you are an established patient please use Rebel Med NW’s online portal to communicate your situation and needs, or leave a voicemail at 206-297-6013 and your physician will contact you at their earliest convenience. Please note that emergency calls over 5 minutes will be subject to a full office fees of a telemedicine visit similar to the regular policy stated under “Contacting Physician”. New patients please contact your established primary care provider.
