Please bring relevant medical history to your appointment or you can send it to us ahead of time. If you’d like us to obtain records from another facility, then you can complete the Records Release form and give it to our office and we will use it to obtain your medical information on your behalf.
Please have your pharmacy send us a Rx refill request. You may need an office visit for a Rx refill if your last medical visit was longer than 6 months ago or you require a new prescription that has not been discussed. It can take up to 2 business days to process a Rx refill, please plan accordingly.
Please use Schedule Online link if you are ready to schedule with a provider. Both new and return patients can schedule online if they know which provider they are interested in seeing.
