We understand that the medical system itself can be a significant source of stress and we take our patient’s resources such as time and finances into consideration.

At Rebel Med NW, we do our best to be candid and transparent in our policies as we build our relationship with our patients. We are your guide and advocate for trustworthy health information whether it is mundane bureaucratic details of the medical system, understanding the current circumstances around Covid-19 and Sars-Cov-2, or finding the profound and lasting health changes that you are looking for.

First Office Visit

Please arrive 15-20 minutes early

New patient paperwork includes the Patient Demographic Form, Patient Intake Form, and Patient Responsibility Form, which requires about 15 minutes to fill out. You will receive an email with the digital forms to complete before your initial appointment when an appointment is scheduled 24 hours or more in advance. If you schedule in less than 24 hours between your appointment, please arrive 15-20 minutes early to fill out your paperwork ahead of time. If you need assistance completing the forms for any reason, please arrive 15-20 minutes early for your appointment to complete the medical forms.

Please bring any relevant medical records along with you on your first visit.  A release of records form is required to obtain records from other healthcare providers.  The form is ideally filled out before or immediately following your first office visit.  The process to receive medical records from your previous or current provider can take up to 2-3 weeks, so please plan accordingly. Any delays may delay the appropriate course of treatment. If you cannot access this form digitally, please come into the office and complete one in person.

If you have any forms you would like the physician to evaluate before your visit, please contact our office. You are required to activate your Patient Portal when you initially register as a patient. This portal has a secure messaging system, and you will be able to upload your forms securely.

How it works

Office visit fees are determined by a combination of the complexity of the visit, procedures performed, and time spent by provider on your case in visit and out of visit.  Because of this, we will offer a range of price estimates prior to a visit, as a good faith estimate for your care.

Additional procedures and codes may be used depending on the complexity of the chief complaint and what the practitioner deems appropriate to achieve the desired health goals of the patient. However, it is important to understand that not all procedures are covered by your insurance plan. We can assist patients in this process and help patients with understanding their insurance policies and financial responsibility.

Common additional procedures and associated fees are standard medical procedures and are priced accordingly. Some examples include blood draws, vaccinations, vitamin injections, in-house lab testing, outside lab fees, etc. Other procedures are more comprehensive and require specialized medical knowledge, including acupuncture, bodywork, and spinal manipulations. Rebel Med NW has chosen to structure itself as a hybrid/concierge practice so that we can continue to sustain our clinic operations as a small business, respectfully pay our providers and support staff, and continue to be here as a comprehensive health resource for our community.

Not every insurance company provides benefits for all services as performed by our providers, as insurance focuses on this disease-centric model for healthcare. Rebel Med NW focuses on adjunctive primary care with a preventative, mind-body, and functional medicine approach to health and wellness.  We find that this saves you time and money, as a true investment in your health and well-being.

How it works

Rebel Med NW offers three pricing models that can be combined to find the right financial program for you. The medical system can result in complicated billing structures, so we try to be completely transparent with our billing. Let’s find the right path for you.

First, let’s look at your health goals. Review if your procedures are covered by your insurance with our list of non-covered services. Then review if direct pricing is a better option for you. We offer discounted rates for our patients with direct payment and concierge membership.

Supplements, Herbs, & Prescriptions

Supplement Prescriptions

Prescriptions for nutritional supplements by a Naturopathic Physician at Rebel Med NW are not charged sales tax* if they are deemed medically necessary for your care. If a prescription refill has run out or expired, a visit or phone consult is required to determine if the nutritional supplement is still medically necessary. At Rebel Med NW, we believe it is important to have an awareness about what you are taking to minimize the use of excessive or unnecessary supplements and to make sure you are taking the safe, researched, and therapeutic dose that is best for your care.

For Seniors age 65+ and for Medicaid or Income-based Adjustment patients, Rebel Med NW offers a 15% discount on supplement purchases made in office. If a Rebel Med physician has prescribed an item, it should be available on one of Rebel Med NW’s online dispensaries.

*Recommendations from acupuncturists, nutritionists, and massage therapists are not applicable for sales tax exemption.

Concierge Membership

**Members of the Rebel Med NW Membership Program do not need an Office Visit. Please initiate an Rx refill via your pharmacy and message your provider through the patient portal. For information about our Membership Program, please contact our office.

Medication Refill Requests

  1. Please have your pharmacy send us an Electronic Rx refill request by reaching out to them first to determine if you have any refills, this may initiate an electronic request from the pharmacy for the provider to approve if you are out of refills.
  2. You may need an office visit for a short Rx refill if your last medical visit was longer than 6 months ago or you require a new prescription that has not been previously discussed. It can take up to 2-3 business days to process a Rx refill based on provider schedule availability, please plan accordingly due to both pharmacy supply, staffing by pharmacy, and hours open. We have recently seen many pharmacies having challenge with staffing and medication availability.
  3. If urgent, please alert our office after the above has been initiated, and may require a $50 non-visit pharmacy fee to initiate the RX by the provider after they review your file.
  4. Consider setting up a 12 month prescription refill with your provider during your annual wellness exam if looking to have Rebel Med NW manage your prescription medicines.

Communications

We communicate exclusively through our patient portal. This ensures that patients’ concerns are addressed in a timely manner while respecting our physicians’ time and ensures the security of patient privacy. New patients and new treatment authorizations require a full office visit. Any phone calls with a physician or provider require the use of that person’s medical knowledge and medical decision-making therefore, phone calls are considered a Telemedicine Visit and will be billed as such.

When You Can't Make it into the Office

Since the 2020 Covid-19 Pandemic, now more than ever, telemedicine is a vital force in patients being able to access their doctors when they need them.

Rebel Med NW utilizes one unified platform for communication to ensure secure patient communication and interaction. Telemedicine provides improved medical care while saving you time and headaches from traveling to the doctor’s office. Imagine not having to travel in traffic or risk exposure to Covid-19 and still being able to get direct medical advice, save time and money, and feel better faster.

Invest in your health by working with a licensed naturopathic physician that can answer many common health conditions, order labs, prescribe medications, or research safe herbal or nutritional supplements to address your health concerns.

Medical Records & Forms

Process & Fees

Your medical records are kept in the strictest confidence in accordance with the Health Information Personal Privacy Act (HIPAA). No individuals have access to your medical records without your prior written authorization.

Any medical records cannot be released until a signed Medical Records Release Authorization form is submitted. Please note that although the staff at our office will do their best to fulfill any medical records request as quickly as they can, it can take some time. Please allow up to 30 days for medical records to be processed and plan accordingly.

At Rebel Med NW, we do our best to accommodate all patient requests in a timely fashion. However, medical bureaucracy takes time and resources. Therefore, we do have a Form Processing fee in place for medical records to lawyers, cert

The processing fee covers administrative costs, costs for duplicating pages, and protecting your privacy when maintaining and transferring medical records. This fee is applied to all forms processing orders such as sending out medical records requests, filling out medical forms such as Family Medical Leave (FMLA) forms, or any other administrative forms.

When requesting Document Processing in conjunction with an Office Visit:

  • $28 administration flat fee
  • Additional $1.24 per page applied to the first 30 pages
  • $0.94 per page beyond 30 pages
  • This is in accordance with the WAC 246-08-400 for form processing fees.

Medical Letters and Admin Forms processed without an office visit have a flat fee of $50.
